IMA Medical Group is seeking to hire a full-time Care Manager to facilitate the process flow for Transitional and Complex Care Management.

A DAY IN THE LIFE OF AN CARE MANAGER

  • The Care Manager is responsible for establishing a systematic process and criteria to identify patients who may benefit from care management. Responsibilities include but are not limited to: implementing a comprehensive, ongoing, and coordinated approach to achieving desired outcomes for a population of patients based on disease groups: i.e., CHF, COPD, DM, Cancer, etc.
  • Perform telephonic Care Management, which includes patient clinical assessment, education regarding disease and early symptom identification.
  • Participate in the identification, collection, and analysis of data for potential enrollment of members to the Disease Management program.
  • Apply critical thinking skills and problem-solving techniques to the disease management process.
  • Accurately conduct a thorough and objective holistic assessment of the patients' current status, including psychosocial, physical, financial, and educational needs, prognosis, and past and present treatment course and services.
  • Partner with physicians for effective clinical management and utilization of resources for patient population.
  • Provide clinical direction and coordination to an interdisciplinary team to formulate and implement a plan of care.
  • Maintain current knowledge of disease processes, available resources, and treatment options appropriate to patient population.
  • Consistently document all activities according to clinical guidelines.
  • Continuously demonstrate a positive attitude and understanding of care/disease management and departmental objectives.
  • Consistently demonstrate excellent communication and interpersonal skills with supervisor, care managers, physicians, office staff, and corporate personnel.
  • Identify opportunities and readily participate in performance improvement activities.
  • Act as a liaison for patients as needed.

QUALIFICATIONS

  • High school diploma or General Education Degree (GED).
  • College coursework in Healthcare is preferred.
  • One year of related experience in outpatient clinic, hospital setting, insurance, medical records, billing or related area preferred.
  • Six to twelve months of data entry experience required.
  • Intermediate level MS Excel skills is required.
  • Working knowledge of medical terminology preferred.
  • Experience with hospital portal systems preferred.
  • Effective interpersonal skills is required.
  • Good organizational and time management skills.
  • Effective Communication Skills (Written and Oral)
  • Must be able to Multitask.
  • Should possess good Phone Skills.
  • Must be Detailed Oriented.
  • Should be a Team Player.
